Desert View Hospital’s medical records department is committed to confidential and safe record keeping. The department fully complies with the Health Insurance Portability and Accountability Act (HIPAA).
If you would like a copy of your medical records, please download and complete an Authorization for Release of Medical Records form. This form must be completely filled out, signed and dated by the patient or representative. You may fax the form or return the form to the facility in person. Please allow 2 working days.
